- Court of the pending action;
- Judge assigned (or Standing Master);
- Type of action (e.g. parenting plan, divorce with or without property, landlord-tenant conflict, neighbor conflict, child-parent conflict, or other type of conflict).
- Deadline for Mediation.
- The names of each party, and the names, phone numbers and e-mail addresses for each counsel.
- How the mediator fee will be split and/or if you would like to be considered for sliding scale eligibility.
